Facilities Use Requests

A facilities use application is required for all facilities use requests.
To complete the Facilities Use Application
  • You will be redirected to a SignNow webpage after you click Submit.
  • IMPORTANT: It may show up as a pop-up, so please check your pop up blocker, if a SignNow webpage does not appear.
  • Enter the appropriate email address and Send Invite.
  • Check your inbox for an email from forms (at myc3churchpmq.net) via SignNow (you may have to check your spam folder) and follow the instructions in the email to access and complete the required registration forms.

Once a form is submitted, no additional action is required on your part. You will be contacted directly regarding the next steps.
